Fecal impaction visits are only increasing as our country faces a mounting prescription opiod and heroin crisis.

Opioid-associated FI can occur in ambulatory seniors, as well. At least 5% of hospital admissions for the elderly are attributed to adverse drug effects (ADEs).5 An emergency department (ED) visit may create more stress for an elderly patient; polypharmacy and ADEs may affect a senior’s presentation, rendering it less apparent; and many elderly patients are hospitalized after they are evaluated in the ED.5 When patients are transferred to or from a hospital, medications are likely to be added or changed, leading to an increased risk of ADEs.

The problem with using a finger to try to scoop out an impaction is that it is not effective or efficient. Each time we make the “J hook” we are distending the bowel and making that patient miserable. The current procedure has many limitations.

Embarrassing for the patient, caregiver and family, digital removal can be very uncomfortable and painful for the patient.

Routinely the clinician needs 5 – 15 passes with their finger to break up an impaction, sometimes taking hours with significant breaks needed for the patient's comfort.
